Meeting Registration Information Complete the registration form to register for the meeting and to indicate your specific session preferences. Register online or mail the completed registration form with payment to the CCA. Registrations are not processed without the appropriate payment. Online registration with credit card payment (VISA and MasterCard only) is available via the CCA web site. Within four weeks of receiving your registration form and payment, the CCA issues confirmation that the CCA received your completed registration and payment. It also provides you an opportunity to verify the details of your registration. We strongly recommend registering for the meeting in advance since space in the sessions is limited. The CCA would like to receive all registration forms by 9/18/09 as sessions are reserved on a first-come/first-served basis. Cancellation Clause In the occurrence of unforeseen events, the CCA reserves the right to cancel the meeting. Meeting fees charged by the CCA office will be refunded. However, the CCA will not be held liable for any airline, hotel, or other penalties that may be assessed to attendees as a result of this cancellation. Meeting Attendees The registration fee for meeting attendees includes the following:
